1. Insert the name and address of the person to whom the notice is addressed.
2. Insert the name and address of the person or water services authoritymaking the complaint.
3. Insert details of the alleged failure referred to.
4. Insert the address of location where the alleged failure is occurring.
5. Insert details of the District Court sitting hearing the complaint.
6. Insert the location, date and time of hearing.
7. Insert the signature of the complainant or authorised person.
8. Insert the date on which the notice is served.

These Regulations prescribe the form of notice to be served by a water services authority or any person intending to complain to the District Court concerning an alleged failure by a third party to comply with a duty of care under
section 70
of the
Water Services Act 2007

(No. 30 of 2007), and for the purpose of giving effect to
section 70
(7) of the
Water Services Act 2007
, hereby make the following Regulations
