If the application is made by the personal representative of a deceased contributor, this form should be accompanied by a form 1.51, duly completed.

The Minister for Industry and Commerce by virtue of the powers conferred on him by section two of the

Unemployment Insurance Act, 1926

, and of all other powers enabling him in that behalf, hereby makes the following regulations :—
