Formats of applications for issuing alien’s residence permit card, for issuing of identity card, for issuing of alien’s passport, temporary residence permit and work permit, and for extension of temporary residence permit and work permit, and for issuing long-term residence permit, as well as format of application for resumption of long-term residence permit

To establish

1) The format “Applications for issuing of a residence permit card, for issuing of an identity card, for issuing of an alien’s passport, a temporary residence permit and a work permit, and for extension of a temporary residence permit and work permit” (Appendix 1);

2) The format of the additional form “Application for issuing a temporary residence permit or for extending a temporary residence permit” (Appendix 2);

3) The format “Application for issuing a long-term residence permit and of application for resumption of a long-term residence permit” (Appendix 3).

To declare invalid the regulation No. 27 of the Minister of the Interior of 14 July 2010 “Formats of applications for issuing alien’s residence permit card, for issuing of identity card, for issuing of alien’s passport, temporary residence permit and work permit, and for extension of temporary residence permit and work permit, and for issuing long-term residence permit, as well as format of application for resumption of long-term residence permit” (RT I, 15.12.2010, 28) as invalid.

The regulation enters into force on 19 June 2011.
