Art. 4

1 Die gesetzlich vorgeschriebenen Zivilstandsformulare wer-

den den Zivilstandsämtern auf Rechnung der Gemeinde durch den

Staat geliefert.

2 Formulare zur Anzeige zivilstandsrechtlicher Vorgänge werden

den Anzeigepflichtigen vom zuständigen Zivilstandsamt unentgeltlich

zur Verfügung gestellt.
