1 Dans sa demande, la victime fournit les indications nécessaires on les circonstances de l'infraction, les motifs justifiant une indemnisation ou une réparation morale, sa situation personnelle et financière ainsi que le montant réclamé; elle mentionne, le cas échéant, d'autres procédures administratives ou judiciaires engagées en relation avec l'infraction dont elle est la victime.
2 La victime joint les pièces utiles à sa demande, notamment les documents qui attestent son revenu.
